Every camper needs these three forms, completed, each summer.
Insurance
Your camper’s health insurance information and waiver.
Camp release
The camp release and attestation, signed by a parent or legal guardian.
Uploading them
- Complete all three forms for each camper, every summer.
- Upload a scan or clear photo of each when you register, or later from your family dashboard — any time before camp.
- If any are still missing as camp nears, we’ll email you a reminder.
- Please don’t email completed forms: they include your camper’s medical information.
